智能障碍智能（或称智力）一般是指接受和运用知识（进行智慧活动）的能力。因智慧活动不是一个简单的心理过程，而是牵涉到感觉、记忆、判断等一系列因素的复杂过程，人类的智力高低差异很大，但多数人的智力处于平均水平。智能障碍是指智力低于平均水平而言。是精神发育迟滞的主要症状。19世纪末期人们开始应用智力测验来测定智能的高低。目前国际上较流行的是两种测验方法，即斯坦福－比奈智力测验和韦克斯勒智力测验，测验结果算出一个指数称为智商（IQ，智力商数）。正常人群的智商呈常态曲线分布,多数人的智商为.100±15,智商低于70（平均值100减两个标准差2×15）便称为智能障碍,高于130者属于超常或天才。智能障碍又可根据其严重程度分为四级：轻度者智商为50～69，中度者智商为35～49，重度者智商为20～34，极重度者智商在20以下（据《国际疾病分类》第九版）。正常智能的基础是脑和合适的学习机会和时机，因此引起智能障碍的主要原因是脑的各种疾病及妨碍学习的各种条件。学习时机很重要，例如，如果在学龄前期（六岁以前）没有学习语言的条件，长大后就很难学会说话（指母语，不包括第二种语言）。瑞士心理学家皮亚杰的研究，认为儿童的智能发育依次可分为四个阶段，即感觉运动阶段，操作前阶段，具体操作阶段和形式操作阶段，这一研究结果受到广泛的引用和承认。智能的全面减退称为痴呆，如果是遗传性疾病或胎儿期疾病引起的，称为先天痴呆；如果是大脑发育成熟之后（通常指18岁以后）再患严重疾病引起的，则称为后天痴呆。曾将先天性痴按大脑功能发育程度和智能缺陷程度分为鲁钝、痴痴愚、白三等（见精神发育迟缓）。后天性痴呆又称急性痴呆（见于急性感、中毒、外伤、缺氧等所致的脑病，可能显著好转）和慢性痴呆（见于慢性发展的脑损害或变性病，不易恢复）。以上两类合称真性痴呆。有一种假性痴呆，表现为意识障碍伴发智慧活动暂时失常，起病急，持续短，消失快，不是真正的智能障碍，但智能的缺陷程度比真性痴呆还严重，多因精神创伤引起,可有以下类型：①癔病性假性痴呆,遗忘既往经历，不知道自已的姓名，不认识亲人，最简单的智能活动全部丧失，持续时间短，可完全恢复，见于癔病。②甘瑟尔氏综合征，答问似是而非,行为荒廖,如把钥匙倒过来开门等，给人开玩笑的感觉，见于监狱精神病和癔病。③童样痴呆，精神活动回到童年，行为言语带有稚气，见于癔病。
