肝阳上亢证肝阴不足，与肝阳失去平衡协调，肝阳相对偏盛，浮动上亢所引起，临床以眩晕，头目胀痛，面红目赤，头重足轻为主要表现的证候。多见于头痛、眩晕、中风、耳鸣耳聋，以及西医的高血压、脑出血、脑血栓形成等疾病。肝阴与肝阳，相互依存，相互为用，对立统一，保持着相对平衡的协调稳定状态，从而维持肝的正常生理功能。若情志不遂，肝郁化火，火伤肝阴，或房劳过度，损伤肝肾之阴，或热病耗伤肝阴，则可导致肝阴不足，不能与肝阳保持相对平衡，如是则肝阳相对偏盛而浮动上亢，出现肝阳上亢证。关于肝阳上亢早在《内经》中即有"阴虚而阳盛"、"肝气上从"等论述。以后，历代医家亦有说明。由于肝阴须靠肾阴的滋养，按十天干及五行配属脏腑，肝为乙木，肾为癸水，故有"乙癸同源"之说。若肾阴不足，则肝阴亦亏，阴不制阳，则肝阳亢盛而为害，故肝阳上亢又称为"水不涵木"。肝阳上亢证的临床表现，以眩晕耳鸣，头目胀痛，面红目赤，急躁易怒，头重足轻，舌红，脉弦有力为主。可兼见口干舌燥，心悸健忘，失眠多梦等症。肝阴不足，阴不制阳，肝之阳气升浮亢逆为此证基本病理变化。肝阴不足，肝阳无制而亢逆，则眩晕耳鸣，头目胀痛；阴阳失调，肝失柔顺之性，故急躁易怒；肝阳亢于上为上盛，阴液亏于下为下虚，上盛下虚，所以头重脚轻，步履不稳；肝阳亢盛则舌红、脉弦有力。若病情进一步发展，肝阳化风形成肝风内动证，可见手足震颤、抽搐或筋惕、肉，或手足蠕动，甚则卒然昏倒等。肝阳上亢证的治疗，以平肝潜阳为法，常用天麻钩藤饮之类。若肝阳化风，可用镇肝熄风汤之类治之。
