脾阳虚证脾阳亏虚，温化不足所引起，临床以脘腹冷痛、肢冷、泄泻为主要表现的证候。由于阳虚易生内寒，有畏寒肢冷等寒象，故又称"脾虚寒证"、"中焦虚寒证"。此证常见于泄泻、痢疾、胃脘痛、水肿，以及西医的慢性胃炎、慢性肠炎、溃疡病、慢性肾炎等疾病。关于脾阳虚证，金元时期李东垣曾以脾胃阳气不足为立论依据，自创升阳益胃汤、补中益气汤、沉香温胃丸等有效方剂。清代叶天士在《临证指南医案》中记载了此证的各种证治案例。脾阳虚证多因平素过食生冷，寒凉损伤脾阳，或久病损伤脾胃阳气，或寒湿之邪困阻脾胃，损伤脾阳而致。脾阳虚的临床表现为脘腹疼痛，喜温喜按，畏寒肢冷，喜热饮，大便清稀，倦怠神疲，纳食减少，或泛吐清涎，或浮肿，或妇女白带量多而清稀，舌淡胖或有齿痕，苔白滑，脉沉弱。脾阳不足，不能温煦脘腹四肢，则畏寒肢冷，脘腹疼痛，寒得热散，故疼痛得温则减，且喜热饮。脾阳不足，运化水谷精微及水湿作用减弱，水湿不化，清浊不分，故大便清稀，或浮肿，带下增多。脾阳不足，胃阳亦虚，故纳食减少，泛吐清涎。气与阳同类，阳气不足，则倦怠神疲。脾阳虚证是脾气虚证进一步发展的结果，脾气虚证失治误治，气虚严重，累及于阳，即可形成脾阳虚证，两者临床均有神疲倦怠，纳食减少等表现，但脾阳虚因阳气不足，失于温煦，往往伴有畏寒肢冷、脘腹冷痛等寒象。脾阳虚证的治疗原则是温中健脾，临床常用理中汤、实脾饮等。
