磺胺脲类降糖药磺胺脲类降糖药一类口服降血糖药物,常用的甲苯磺丁脲(简称甲磺丁脲、甲糖宁、D860)、氯磺丙脲、优降糖等。本类药物对正常人和糖尿病患者都有降血糖作用。主要是直接刺激胰岛β细胞释放胰岛素,使内源性胰岛素增加,产生作用。反复用药可使胰岛β细胞增生,胰腺重量增加。对胰岛功能丧失者无效。甲磺丁脲口服吸收快,半衰期约5小时,作用维持5~12小时,临床最为常用,严重慢性肝病者禁用;氯磺丙脲口服吸收快而完全,半衰期25~40小时,作用维持达60小时,有严重肾脏疾患时禁用;优降糖,口服吸收完全,半衰期4~8小时,作用能维持15小时,作用快而强,比甲磺丁脲大100~200倍,故用量应小。此类药物适用于成年型非胰岛素依赖型糖尿病患者,单用饮食控制无效而胰岛素需量在40单位以下的中,轻型病人。氯磺丙脲具有抗利尿作用,故可用于治疗尿崩症。常见的不良反应为胃酸分泌增多,引起恶心、腹痛、腹泻、偶见皮疹、药热、皮肤红斑、过敏性皮炎和荨麻疹,优降糖还引起低血糖反应。大剂量氯磺丙脲可引起精神错乱、眩晕、共济失调、无力等。
